Now into Court comes the defendant herein, through undersigned counsel and on suggesting to the Court that the record herein shows error to his prejudice and that he is entitled to a direct appeal to the Supreme Court of Louisiana and that the Louisiana Public Defender Board [1010 Common Street, Suite 2710, New Orleans, LA 70112-2401] be appointed to appeal this case, pursuant to the mandates of R.S. 15:149.1.

    FURTHER, pursuant to C.Cr.P., Art. 914.1(A), the defendant respectfully designates the entire transcript of the trial, both the guilt and penalty phases, including all of the testimony, the voir dire examinations, all opening and closing statements, all arguments or hearing out of the presence of the jury and the jury instructions, and the entire transcript of each and every hearing, including any pre-trial and post-trial motion hearings and the sentencing, herein and all of the pleadings for inclusion in the appellate record.

    WHEREFORE, he prays that he be granted an appeal to the Supreme Court of Louisiana, returnable in accordance with the law.
